Going Home
Your doctor will order your discharge from the hospital and provide you with specific discharge instructions. A nurse will review your instructions with you before you are discharged. Please ask as many questions as you need to about your discharges instructions. Discharge time is noon.

Check your room before you leave to insure no belongings are left behind. When you are ready to leave, an escort will take you to the Union Hospital West lobby.

Discharge Planning and Medical Social Services
Social workers and other professionals are available to help you and your family plan for your care after you leave the hospital. They offer a variety of services and can help coordinate community services to assure continuing care when you are discharged.

Medicare Counseling Services
The Medicare Volunteer Corps is available to assist with insurance billing and answer questions about insurance coverage, supplemental and long-term care insurance as well as helping your resolve billing problems. To request this service, dial extension 5325 or 7588.
