Discharge Planning


Discharge plans will be discussed soon after admission and throughout your stay. Our primary concern is to assure a smooth transition to the most appropriate setting.

Indicators used to decide that you are ready for discharge include:

  • The patient has met goals established at, and subsequent to, the time of admission.
  • The patient requires only one rehabilitation service.
  • There is evidence of failure to show improvement in function when discussed by the rehabilitation team.
  • A complicating medical condition develops which causes suspension of rehabilitation therapy. Each patient will be discussed on a case-by-case basis by the rehabilitation team for length of stay.
  • The patient demonstrates non-compliance or failure to participate in his/her rehabilitation program.
  • The family demonstrates non-compliance or failure to participate in the rehabilitation education program.
  • The needs of the patient could be best served in another setting. (i.e., sub-acute, long-term care or acute care setting)

Discharge planning procedures are a team activity that require communication between you, your family and your other rehabilitation team members. Concerns about discharge or changes in decisions regarding discharge should be brought to any team member.
